Dave Bascombe said in the 2006 Music For The Masses Remaster DVD documentary:

"It was actually an accordion with just no keys held down. There's a button you press on the accordion which lets all the air out. And that's all it was. But it does sound really disgusting. It's great! Well, that song has got some heavy breathing on it anyway, which we sampled from God knows where - I think we actually sampled it from a porn movie, and then we got some girls in to add to it, to make it a bit more hi-fi or something."
